A W9 form is required by the IRS for a 1099 position.
After downloading the form, follow the form instructions and fill out as instructed. Please check the accuracy of your details. Email it to us at restaurants@culinarysea.com and a team member will reach out to you.
Download the Culinary Sea App
After receiving your information we will forward a link to download our driver app where you can create a profile and join our Fleet. Once you are approved you will start receiving orders when you go online

Required: A valid driver's license and insurance
Applications without a W9 form will NOT be accepted. If you deliver by car, you must be 19 years old or older, have a car and a valid driver's license
